La tradizione vuole che la chiesa sia stata edificata per volontà di San Francesco di Assisi in occasione di una sua sosta a Mondavio nel XIII secolo.L’assetto attuale corrisponde ai rifacimenti del XVIII secolo, che hanno conservato,l’austerità e la sempilicità che hanno sempre contraddistinto gli edifici dell’Ordine Francescano. L’interno della chiesa conserva opere pregevoli. Tra tutte la più importante è una tavola raffigurante l’immacolata concezione del pittore fanese Giuliano Persciutti.
